I acknowledge the registration process is endeavoured to be completed in 3 working days once the form has been received by Triage Vets, however, missing information could delay this.

I understand the importance of ensuring previous veterinary medical records have been transferred to Triage Vets prior to my appointment. I am aware this is beneficial for my pet’s continuation of care and is strongly recommended.

Insurance: At Triage Vets our treatment to pets remains unchanged whether you are insured or not. Insurance is an agreement between you and your policy provider, nothing to do with or related to the care we give. However, we can definitely be testament to the fact that having your pet insured can give you a peace of mind. Having a sick or injured pet is worrying enough, without having to worry about the cost of treatment. Today, advances in the veterinary profession can offer a vast amount of options to your beloved pets, the last thing you want to do is give your pet anything less than the best care available. Make sure your insurance provider covers all the bases:

· Does the policy provide ‘life-long’ cover for on-going conditions?
· Are vet benefits ‘per condition’ or is there a maximum amount for all conditions?
· Do benefits ‘re-charge’ every year that the policy is in force?
· Does the excess need to be paid once per condition or annually per condition?
· What is the excess? What is the co-payment and when does it apply?
· Besides cover against vet fees, what else does the policy include as standard?
· What is your “cooling off” period before you can visit your vet.

I acknowledge by visiting before the 'cooling-off' period, any concerns or issues raised in the health check or routine appointment could be deemed as pre-existing conditions by my insurer.

I understand that Triage Vets are kindly willing to assist me to make claims from my insurance provider, but are not responsible for the upkeep of this duty. I must instruct Triage Vets to assist me to serve a claim to my provider knowing it could take up to 10 working days for Triage Vets to complete. Please refer to your insurance policy for payment timelines thereafter.
